Your registration is NON-REFUNDABLE, but fully transferable to another youth if you can no longer attend. Please contact us, ASAP.
If your girl arrives late, leaves early, or is dismissed for any reason, there will be no refund.

PAYMENT OPTIONS
Pay in Full: By selecting Pay in Full, your preferred payment method will be charged the full workshop session cost.
Payment Plan: By selecting Payment Plan, your preferred payment method will be charged a $60 deposit upon registration. The remaining balance will then be divided into equal monthly payments between the date accepted and June 2, 2018.

If you plan to apply for a scholarship, please be sure to apply as soon as possible, as it’s on a first come first serve basis. Once all our available scholarship funds have been allocated, we will be unable to offer any further aid. If you have any questions please feel free to contact us at girls@girlsarepowerful.org.
HOW TO APPLY
Please apply HERE. Additional questions regarding your income will be asked at that time. We may also request additional documents as proof of income, if necessary to make a decision. If awarded, we’ll notify you within 5-7 business days of our decision. You will have one week to accept the award.

RELEASE AND WAIVER OF LIABILITY AGREEMENT
This waiver must be signed by each participant or parent or guardian of the participants before participating in the program.
I understand that, as part of her involvement in the Girls Are Powerful (GRP) program (the “GRP Program”), my daughter may participate in activities including, but not limited to, events, workshops, programming, team-building games, outdoor play, and field trips using approved transportation. I authorize my daughter to participate in the activities of the GRP Program. I agree that GRP may end my daughter’s participation in the GRP Program at any time, at its sole discretion.
I recognize that safe participation in activities requires my daughter’s attention to instructions provided by the GRP Staff. To the extent of my knowledge, I have informed GRP of any conditions that may limit my daughter’s participation. I understand that, while GRP Staff are trained to avoid and reduce the risk of injury, there is no way to eliminate the possibility that my daughter may be injured when participating in the GRP Program. I understand that GRP could not afford to offer the GRP Program if it were required to bear financial responsibility for those risks. As consideration for GRP allowing my daughter to participate in the program, I accept full and sole responsibility for any and all harm, injuries, and losses to my daughter or her property, except those caused by the greater than ordinary negligence of GRP. I hereby release and forever discharge GRP, the GRP Chapter, and their respective directors, officers, employees, volunteers, agents, contractors, attorneys, and representatives (collectively “Releasees”) from any and all actions, claims, or demands that I, my daughter, or either of our assignees, heirs, distributees, guardians, next of kin, spouse and/or legal representatives now have, or may have in the future, for injury, death, or property damage arising directly or indirectly out of my daughter’s participation in the GRP Program, except those arising solely from greater than ordinary negligence of GRP and the GRP Chapter. I agree to indemnify, save, and hold harmless the Releasees from any and all damages, liabilities, and losses arising directly or indirectly out of my daughter’s participation in the GRP Program, except those caused by the greater than ordinary negligence of GRP.
In case of emergency, I authorize physicians engaged by the GRP Chapter during the program year, to provide any medical assistance and treatment that the above-named child may require, even if it is not possible for me to be consulted. I understand that, if not fully covered by a medical insurance provider, I will be personally responsible for all costs incurred for the care provided.
I understand that GRP will evaluate the effectiveness of the GRP Program through surveys and other methods. I give permission for my daughter to participate in the evaluation process. I understand that GRP may take photographs and/or videos of the girls participating in the GRP Program, including my daughter, for promotional or publicity purposes (such as brochures, annual reports, web pages, exhibit displays, newspapers, magazines, or television news).e program’s success. I understand that the material may be edited by GRP. I give permission for my daughter’s photo to be used in promotional or advertising material, including, but not limited to, reports, posters, Internet, and other electronic media. I grant and convey unto GRP all right, title, and interest in any and all photographic images and video or audio recordings of my daughter made by GRP during the GRP Program including, but not limited to, any royalties, proceeds, or other benefits. **If your daughter cannot be photographed for legal or religious reasons, please attach a note**
I agree that this Agreement is intended to be as broad as permitted by the laws of Minnesota. I agree that, in the event that any part of this Agreement shall be held to be invalid by any court of competent jurisdiction in Minnesota, the rest of the Agreement will continue to be enforceable.
I HAVE CAREFULLY READ ALL OF THE INFORMATION IN THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I GIVE PERMISSION FOR MY DAUGHTER TO PARTICIPATE IN THE GRP PROGRAM. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT WITH GIRLS ARE POWERFUL, LLC, AND SIGN IT OF MY OWN FREE WILL.

MEDIA CONSENT & RELEASE
I do hereby consent and agree that The Girls Are Powerful Foundation has the right to take photographs or record video of me and my child / each participant (and/or my property) and to use these in educational, promotional, industrial, and commercial material. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.
I do hereby release The Girls Are Powerful Foundation all rights, claims, or interests I may have to control the use of my identity or likeness in these photographs or video/films and agree that any uses ** described herein may be made without compensation or additional consideration to me.
I represent that I am at least 18 years old, have read and understood the forgoing statement, and am competent to execute this agreement.
